Insurance Information
As a courtesy to our patients, our office will coordinate between any medical and dental insurance, in-network or out-of-network, provide any documentation needed prior to and post surgery, as well as submit and track all claims post surgery.
Your insurance coverage is a contract between you and your insurance company. It is not possible for us to provide services on the basis that your insurance will pay all charges because plans vary greatly, often leaving a portion of payment to be responsible to the patient, if not all costs. For this reason, all insured patients will have a financial requirement due on the day of surgery. You will be informed of this cost prior to your surgery date. We will submit to in and out of network insurance carriers. On average, it can take up to 90-120 days for full processing of both medical and dental insurances. If there is an over payment, a refund will be issued to you from our office. If there is a balance after claim processing, you will be issued a statement. (Click HERE for Insurance Disclaimer Form)
Insurance verification (to confirm the dental benefits of your specific plan) is a convenience our team offers to patients. However, to avoid any misunderstanding on available coverage or in-network vs out-of-network availability, we encourage you to also call your insurance provider to discuss your coverage options prior to scheduling. Your insurance plans are selected and owned by you and you are responsible for understanding your coverage and plan disclaimers. If needed, our office is happy to provide treatment plans to patients prior to their appointments for insurance verification. For patients requiring a very specific understanding of coverage, a pre-authorization is highly recommended.
Please note: Some or all procedures may not be covered by your insurance plan. You are fully responsible for the costs of these services, regardless of insurance estimates.
For any payments that may be required after insurance, or for those who may not have insurance coverage we do accepts personal checks, cash, all major credit cards, HSA/HRA plans, and Care Credit - to see what you qualify for through Care Credit you can apply at: https://www.carecredit.com/.
Should you fail to show or cancel your scheduled surgery appointment without giving a 48-hour notice, you will be subject to a $100.00 cancellation fee. You will then be required to “pre-pay” the full amount of your surgery PRIOR to being put back on our schedule. This is regardless of any insurance plan you may carry. Should this generate an over-payment on your account once we receive your insurance reimbursement, we will gladly send you a refund.
Account balances over 45 days are the patient’s responsibility, regardless of insurance coverage.
Account balances over 60 days are subject to billing charges until paid in full and may be referred to an outside source for collection action. You will be responsible for any such collection and/or attorney fees.
We charge $50.00 for returned checks.
