Please provide the following information prior to your appointment:
- Referral slip (from your dentist) – please text to our office @ 704-659-6765
- Registration forms - filled out at least 48 hours prior to your appointment (CLICK HERE for registration forms)
- Dental insurance – if you have dental insurance, please text a picture of your card to our office @ 704-659-6765
- Any panoramic X-ray images taken within the past six months will be requested by our team from your referring dentist/specialist
During your first visit to our office, Dr. Pete and team will review your medical and dental history, go over your diagnosis, and conduct a detailed discussion of treatment options. A treatment plan will be established as a result of your thorough discussion with Dr. Pete. You will then meet with a member of our team, who will walk you through the financial impact of the treatment plan you and Dr. Pete developed. If you have insurance, we will also discuss your estimated insurance benefit coverage vs the estimated patient out-of-pocket expense.
Occasionally, surgery may be performed on the same day as the consultation. However, this cannot be guaranteed and will be dependent on your health history and treatment plan pathway.
Any oral surgical procedures for children, patients with complex medical conditions or special needs, and patients who are undergoing IV sedation, will require a separate consultation scheduled prior to the surgery.
Please arrive ten minutes prior to your consultation so we can verify any information.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.
